Insurance and Payment:

Most services are covered under Medicare, part B. Prior to your initial evaluation, we will let you know which services may be covered. For all other insurance carriers, we can assist with billing your insurance as an out-of-network provider, and also offer private-pay options. For those using insurance, you may only be responsible for your copay or coinsurance, unless you have a secondary insurance policy. We will verify your insurance prior to your initial evaluation. For those who are paying out of pocket, a 35% prompt pay discount is applied, which is reflected in the pricing below:

Pricing:

  • This includes an expert, comprehensive home assessment, including a written report with all recommendations for home modifications, adaptive equipment and safety education (1.5-2 hours).

    $300

  • Initial evaluation for all rehabilitative services, to gather medical history, assess areas of concern, make initial recommendations and develop a treatment plan. (60-90 minutes)

    $180

  • This covers all follow-up visits for rehabilitation (OT and/or PT), self-care training, and adaptive equipment installation and training. (50 minutes)

    $150

  • Wellness visits are designed for people who met their rehabilitation goals, and wish to continue expert, one-on-one training with a professional. Wellness programs are not reimbursable under insurance. Discounts available for packaged wellness plans (see below).

    • $150 with an OT or PT (50 minutes)

  • Package rates (10% off) are available for those who wish to schedule regular visits with a professional. The discount amounts to $135 per visit as opposed to $150 for a single visit. Packages are paid in full prior to the start of services.
